In this month’s edition of the NOVA SBE Health Economics & Management Seminar Series, we welcome Simone Ghislandi, who will present his latest research on how shocks in the primary care workforce affect healthcare consumption and health outcomes among older adults, focusing on disruptions in continuity of care following GP exits.

Breaking Continuity. The impact of Primary Care Workforce Shocks on Older adults’ Health and Healthcare consumption

ABSTRACT:

As many European countries face growing shortages of general practitioners (GPs), understanding the consequences of primary care workforce instability is crucial for health policy.

This paper estimates the causal impact of GP exit on hospital utilization and mortality in a Beveridge-type healthcare system. Using 2010-2019 comprehensive administrative panel data from the Lombardy Region (Italy) covering all chronic residents aged 65 and above, we exploit GP exits as exogenous shocks to the patient-GP relationship and implement a three-way fixed effects difference-in-differences model.

While previous studies typically include only individual and time fixed effects, our analysis additionally absorbs persistent GP-level heterogeneity to mitigate omitted variable bias. Our findings show that forced physician switching leads to economically meaningful and persistent increases in the probability of hospitalization, Emergency Department (ED) admissions and, importantly, mortality.

Effects are heterogeneous. Stronger responses are observed among men and economically vulnerable individuals, while no significant impact is found for higher-income patients.

Adverse effects are observed mainly in metropolitan areas. Transitions from older to younger GPs generate temporary increases in ED use, consistent with differences in clinical experience requiring short-run adjustment dynamics.

Overall, the results highlight the central role of continuity of care and suggest that GP workforce instability generates sustained increases in hospital utilization, with important implications for resource allocation and equity.

Short Biography – Simone Ghislandi

Simone Ghislandi is Associate Professor in the Department of Social and Political Sciences at Bocconi University, Italy. His research focuses on health economics and policy, public economics, and the evaluation of public policies, with particular attention to health systems and population health.

He is Co editor of the journal Biodemography and Social Biology and Associate Editor of Discover Social Sciences and Health.

Professor Ghislandi holds a DPhil and an MPhil in Economics from University of Oxford and a Laurea summa cum laude in Economics from Bocconi University.
